Statement of Work, Terms and Conditions

In consideration of the monies paid to CaliCo Solutions by the party named above ("Customer"), the services to be performed by CaliCo Solutions include (but are not limited to) the following:

Set up the Customer County in CalCATS, establishing such database records as are necessary for Customer to begin or continue to use CalCATS.
- Database setup includes initial user accounts, progams, subprograms, districts, and business locations. It also includes summary information about prior Weights and Measures Inspections such as last inspection date and number of failed devices.
- Importation of historical time entries, activities, or inspections is not included in the services. Such services can be provided as Professional Services if so desired.
Allow Customer and their Authorized Users to log in to and use CalCATS via the public Internet.
Provide Customer with access to available help resources including how-to videos and the CalCATS Help Desk. The CalCATS Help Desk provides the following support to Authorized Users:
- Support via Email at the help desk email address listed on each page of CalCATS.
- Initial response to issues identified as Urgent within four (4) business hours (Pacific Time Zone).
- Initial response to non-Urgent issues within twenty-four (24) hours.
Train Customer staff in the proper use of CalCATS:
- Training sessions are provided on a regular schedule which may be amended from time to time.
- Initial system training will be available via web meeting or in person.
- Training sessions may include multiple Customer organizations.
- Training materials and video training are available on a self-serve basis within CalCATS.
Operate, maintain, and enchance CalCATS as described in the Subscription Agreement.
Deliver copies of all Customer Data to Customer at any time.

Customer's use of CalCATS and provision of the services described above shall be governed by the CalCATS Subscription Agreement. By submitting this Order Form, Customer acknowledges that they have read and agree to abide by the terms of the CalCATS Subscription Agreement. A copy of the agreement, and the acceptable use policy to which it refers, are available below.
